In their book, "Primed to Perform", Lindsay McGregor and Neel Doshi talks about the tendency to blame people, rather than the systems or environment they operate in -- calling it our "Blame Bias". They do a great job of describing both the dynamics of the situation, as well as the types of leadership decisions and behaviors that result.
